I imagine there's quite a ton of them out there, mainly more obscure and cult films. Though here's some off the top of my head:
Snappy Video BD of Dracula and His Vampire Bride.
Dark Star (Bryanston Pictures logo suggests a faded 35mm blowup print was used and then sandblasted with degraining).
The Last Valley (IP was in abysmal shape and Kino reportedly used two different 35mm prints, one from the BFI and another being Joe Dante's personal copy).
Umbrella BD of Body Melt (their 4k transfer was based on a battered 35mm Aussie release print that rights holder Beyond Entertainment had. Vinegar Syndrome later found the 16mm OCN somewhere in Canada and did a 2k scan/restoration of that. Their transfer was used on the VS Blu as well as the UK/German BDs).

Japanese Extended cut of Police Story (found on the Eureka UK Blu as an extra).
Hong Kong theatrical cut of Police Story II (found on the Criterion Collection Blu as an extra).
The Leg Fighters.
Dynamo, known as Bu Ze Shou Duan (both HK and shorter cuts are sourced from a combination of various theatrical prints and one loaned from Quentin Tarantino's personal collection).
Japanese extended cut of Heart of the Dragon, titled The First Mission (can be seen in it's entirety on the Japanese Shochiku Blu. Note the UK 88 Films Blu has this version reconstructed with bulk coming from Fortune Star remastered footage of the HK theatrical cut).
Japanese extended cuts of Fists of Fury, Way of the Dragon, and Game of Death (the former two are only complete on the Japanese BDs as extras, while the US Shout Factory Blus have the latter in full as an extra).
German BD of The Reflecting Skin (comes from German dubbed and localized 35mm theatrical print).
German BD of One Eyed Jacks (WARNING: horrible PAL DVD upscale from battered French theatrical print).
